Accurx Triaging
As of 17 February 2025 the practice have moved to Accurx as our triaging tool.
The Accurx form is open on the following times (excluding bank holidays):
Monday - 08.00-18.30
Tuesday - 08.00-18.30
Wednesday - 08.00-20.00
Thursday - 08.00-18.30
Friday - 08.00-18.30
Accurx form can be used for:
- Requesting an appointment for a new or ongoing medical problem.
- Seeking advice on symptoms, conditions or treatments.
- Admin requests such as sick notes, blood results, medication reviews etc.
Use the Accurx form above to contact our practice. You can also use Accurx through a web browsers (on laptops, desktops and mobile devices) and are able to use NHS login to verify your identity and pre-populate demographic details when completing. NHS login provides patient verification and opens up opportunities for further integrations involving Accurx. This is only optional you are able to submit an Acccurx as a guest without having to log in.
